چکیده

PurposeAllosensitization is associated with post-transplant complications in heart transplant (HT) recipients. Lymphocyte-depleting induction may mitigate these risks, but efficacy data conflicting.MethodsThis single center, retrospective cohort study compared highly sensitized HT recipients (cPRA ≥50%) receiving lymphocyte-depleting vs. no from 2015-2021. The primary outcome was a composite of high-grade biopsy-proven acute rejection (BPAR) defined as ISHLT ≥2R rejection, and death at 12 months.ResultsOf the 53 patients identified, 25 received (16 anti-thymocyte globulin [rATG] 9 alemtuzumab). Baseline demographics were similar between groups except higher prevalence renal dysfunction group (p=0.002). There significant difference endpoint (HR, 0.16; 95% CI, 0.02-1.34; p=0.09). lower hazard any BPAR 0.27; 0.10-0.76; p=0.01). This driven by rates cellular control arm (p=0.02).ConclusionLymphocyte depleting resulted less confers statistically benefit early or reduction month mortality. Allosensitization conflicting. months. Of (p=0.02). Lymphocyte

Impact of pretransplant rituximab induction on highly sensitized kidney recipients: comparison with non-rituximab group

PURPOSE Highly sensitized patients with a high level of panel reactive antibody (PRA) experience more episodes of antibody-mediated rejection (AMR) and poorer graft survival than non-sensitized patients. Rituximab is a well-known monoclonal anti-CD20 antibody that causes the depletion of B lymphocytes. The aim of this study was to compare a rituximab-administered and a non-administered group of...

Duration of graft cold ischemia does not affect outcomes in pediatric heart transplant recipients.

BACKGROUND Utilizing donor hearts with prolonged graft ischemia may extend the donor pool. METHODS AND RESULTS The medical records of 363 infants and children, aged 1 day to 17 years, transplanted at Loma Linda University between November 1985 and March 2001, were retrospectively reviewed. Fourteen children received organs with prolonged ischemic times (>8 hours)(PIT) compared with 14 with sh...

Socioeconomic position and graft failure in pediatric heart transplant recipients.

BACKGROUND Socioeconomic (SE) position may affect availability of resources, health-related behavior, and outcomes. We assessed whether patient SE position, determined for the block group of patient residence (average population 1000, smallest census unit with SE data), is associated with graft failure in pediatric heart transplant recipients. METHODS AND RESULTS We used the US Census 2000 da...

Approach to kidney transplant in sensitized potential transplant recipients.

More than one-third of patients on waiting lists for kidney transplant are sensitized. Most have previously formed donor-specific and non-donor-specific serum antibodies and/or positive crossmatch by complement-dependent cytotoxity and/or flow cytometry. Two categories of alloantibodies include antibodies against major histocompatibility complex human leukocyte antigen class 1 and class 2 and a...
